Scaling and root planing has a visibility problem.
Calculus on a root surface is not evenly distributed, and it is not always where a tactile explorer says it is. Residual deposits in deeper pockets, on furcations, and along root concavities are the common reason a site that was treated still bleeds at re-evaluation. When you can't see the surface you're instrumenting, you're closing pockets on an assumption.
Perioscope therapy removes the assumption. A fiber-optic endoscope is introduced into the pocket and the root surface is displayed on a monitor while it's being debrided.
